CASE 3
ANSWER: Infantile
hemangioendothelioma of the left lobe of the liver.
Unfortunately the radiologist involved did not consider this, and the patient
underwent lobectomy of the left lobe of the liver. No delayed CT scans
were done. If they had been done, progressive opacification of the central
portion of the lesion would have been seen, pathognomic for vascular tumors
of the liver.
.
. Figure
1. The plain abdominal radiograph shows a large mass in the
left upper abdomen. It has pushed the stomach
to the right and the
transverse colon downward.
Figures 2 and 3. Abdominal ultrasonograms
show an inhomogeneous echogenic
mass in the left lobe of the liver. Note
a small calcification (arrow) at the periphery
of the mass.
Figures 4 and 5. CT scans of the liver show
a hypodense mass with punctate
calcifications (arrows) in the left
lobe of the liver. It is enhanced peripherally
and lobulated.
